Social prescribing is a service available for clinical staff at the practice to refer patients to, this service directs patients to non medical support services within the community. The social prescribers or otherwise known as link workers work with all health care professionsals to address the social emotional and practical needs of patients. The social prescribing team often helps patients with issues, such as social isolation, problems with money, issues with loneliness, mental health etc.
